Are you ever in a first or second session with a client, and just wish you had an outline to articulate exactly what creates a good recipe for successful therapy? Good news; I created that for you. May I present to you; My Recipe for Successful Therapy! *Disclaimer. This does not ensure that therapy will be successful. This lays out for both parties what ingredients need to be present in order for this to be a good fit and order for this process to go as smoothly as possible! As soon as one party recognizes that an ingredient is missing, it’s time for a conversation. As my clients hear me say often (not sure if they’re sick of it yet or not) “clarity is kindness!”
Daunted or scared to start writing your own intake paperwork for potential clients? Have no fear! If you’re anything like I was, you just want to sit down with your clients, do the therapy part of things, and let someone else do the administrative work. That’s where I come in. In this packet you’ll get access to my HIPAA compliant, trauma informed, all inclusive questionnaires that will prepare you to have a successful first session with your perspective clients! Included is: Informed Consent (please personalize with your clinic policies) and patient rights, Symptom Checklist, Patient Demographic Form, Payment Authorization Form, Patient History Form, Fee Agreement, Insurance Form (if you take insurance). Plug the info into whatever your preferred Electronic Records system is, or wherever you store your confidential information, and have a great intake! My intake forms are lots of questions, because the more I know the more I can help and the less surprises there are for them. As I tell my clients; take what you can use and leave the rest!
The first step to thorough trauma work is establishing safety with your client. The top determining factor of successful therapy isn’t how much you know, it’s trust and therapeutic fit! Only when we know we have access to the right tools to create safety in our nervous system, can we begin to do the deep work that is needed in order to heal those core wounds from past trauma. Inside this packet you’ll find a variety of top down and bottom up techniques as well as ideas for writing a safety plan for higher risk clients. They key as you go is to be in communication with your client about what is working and what is not; and document what you utilized and how it went as you go!
